Forms

As with any large organization, the United Church processes, especially Pastoral Relations and personel processes, are driven by forms. This page provides links to some of the most-used forms, so people who need them can download them as necessary. Most of these forms need to be completed and then passed on to the Pastoral Relations committee.

Typically, forms are available in one or both of two formats:

.doc
Editable format, suiable for use with Microsoft Word. Documents in this format can be filled in using a computer, and then e-mailed to the recipient. Alternatively, they can be printed and filled in by hand, then sent on. You must have Word, or a compatible package such as OpenOffice, installed on your computer to use this format. (If there is a demand, we could make these forms available in a format suitable for other word processing systems.)
.pdf
A display format only. Documents in this form generally must be printed and completed by hand. You must have a PDF software package, such as Adobe Reader, installed on your computer to use this format.

If you are not able to work with either of these formats, please contact the Presbytery Secretary and ask for a copy of the form you need to be sent to you.

The following documents are currently available on this web site. Click on the link under the preferred format to download it. Note that to work with a form in .doc format, you must save the file on computer before you can make any changes to it.
